Private RN Consultation and Caregiver Education for a Confident Transition Home
Hudson Valley Private Nursing, HVPN, provides individualized, hands-on nursing care, including wound and ostomy care and medication administration, along with discharge instruction review, caregiver education, recovery planning, and healthcare navigation guidance for families after hospitalization.
Each visit helps families receive needed nursing care, understand next steps, prepare for follow-up care, and feel more confident managing recovery at home.
Care is provided personally by Andreen Robinson, MSN, RN. Available by appointment and private pay, based on family needs, RN scope of practice, provider orders where required, availability, and safety considerations.
HVPN may be appropriate when a medically stable family needs additional RN education, caregiver preparation, discharge instruction support, or help organizing questions and next steps after discharge.

How HVPN supports your patients and clients
- The patient and family need hands-on nursing care or caregiver education that was not provided during discharge
- The situation involves wound care, ostomy care, or complex medication changes that call for direct nursing attention
- Private, appointment-based nursing care is clinically appropriate and within individual RN professional practice scope
- The family has chosen to pursue private nursing care and will make direct contact with HVPN independently
- The patient and family do not require continuous nursing coverage, staffing, or agency-level home care

A simple, transparent process with no clinical information required upfront
Request a referral conversation
Use the form below or call directly. General context about the type of transition or education need is sufficient, no patient names, records, or protected health information through this form.
Discuss referral fit
A brief professional conversation to confirm whether HVPN’s services are appropriate for the situation. If the need falls outside HVPN’s scope, that will be communicated clearly and appropriate licensed alternatives will be suggested.
The patient or family contacts HVPN directly
HVPN does not initiate contact with patients through referral partners. The patient or an authorized family member contacts HVPN independently to request a consultation. This supports patient choice and appropriate privacy boundaries.
HVPN obtains authorization before discussing clinical details
Required patient authorization is obtained before HVPN discusses protected health information with any provider, including the referring professional.
Scope review and service engagement when appropriate
HVPN confirms clinical fit and scope appropriateness before beginning any service engagement. Referral partners receive professional communication when authorization has been obtained and the engagement is underway.
