Skilled RN Care for Patients Who Need More Support After Discharge

HVPN provides individualized nursing care at home for selected patients who need clinical support, education, and coordination after hospitalization, surgery, or a change in health status, including wound and ostomy care, medication administration, and discharge instruction review.

Care is provided personally by Andreen Robinson, MSN, RN, by appointment and private pay, based on family needs, RN scope of practice, provider orders where required, availability, and safety considerations.

How referral discussions work

A simple, transparent process with no clinical information required upfront

1. Discuss fit

Contact HVPN to discuss whether the patient’s needs may be appropriate. No PHI is needed for an initial conversation.

2. Connect the patient

Provide the patient or authorized family member with HVPN’s contact information. HVPN does not initiate contact; they reach out to us directly.

3. HVPN handles intake

HVPN obtains required patient authorization before discussing any clinical details with you, then evaluates fit and coordinates appropriate care.

Important: HVPN does not pay or accept compensation in exchange for referrals. All referral arrangements are professional courtesy and based solely on appropriate service fit for the patient and family.
