Skilled RN Care for Patients Who Need More Support After Discharge
HVPN provides individualized nursing care at home for selected patients who need clinical support, education, and coordination after hospitalization, surgery, or a change in health status, including wound and ostomy care, medication administration, and discharge instruction review.
Care is provided personally by Andreen Robinson, MSN, RN, by appointment and private pay, based on family needs, RN scope of practice, provider orders where required, availability, and safety considerations.
Refer, and HVPN takes it from there.
Consider HVPN when a medically stable patient or family needs additional RN care, caregiver preparation, discharge instruction support, or help organizing questions and next steps after discharge.
Service area: Proudly serving patients and families throughout Orange County and the Hudson Valley.
