How we use it

• To respond to your inquiry: contacting you about the care or referral request you submitted
• To coordinate care: when you've asked us to, communicating with the hospital, physician, or care manager involved in your or your loved one's care
• For internal records: keeping a basic log of inquiries so nothing falls through the cracks

How it's protected

This site is served over an encrypted (HTTPS) connection. Form submissions are processed directly by our website and email system, we do not route form data through third-party marketing or analytics platforms. Consistent with New York's data security law (the SHIELD Act), we maintain reasonable administrative, technical, and physical safeguards appropriate to the size and nature of our practice.

Data retention

We retain inquiry information only as long as needed to respond to you and, where a care relationship begins, as required by New York nursing practice recordkeeping standards. You may ask us to delete information related to an inquiry that did not result in care by contacting us using the information below.

• You are never required to submit detailed health information through this website, phone is always available as an alternative
