When you use the contact or referral forms on this site, we collect what you provide directly: your name, phone number, email address, and any message you choose to include. We intentionally keep these forms brief and ask that detailed medical history not be submitted online, specifics are gathered by phone once we connect.
We do not use third-party advertising trackers, analytics scripts, or session-recording tools on our contact or referral pages. We do not sell, rent, or share your information with third parties for marketing purposes.
How we use it
- To respond to your inquiry: contacting you about the care or referral request you submitted
- To coordinate care: when you've asked us to, communicating with the hospital, physician, or care manager involved in your or your loved one's care
- For internal records: keeping a basic log of inquiries so nothing falls through the cracks
How it's protected
This site is served over an encrypted (HTTPS) connection. Form submissions are processed directly by our website and email system, we do not route form data through third-party marketing or analytics platforms. Consistent with New York's data security law (the SHIELD Act), we maintain reasonable administrative, technical, and physical safeguards appropriate to the size and nature of our practice.
Data retention
We retain inquiry information only as long as needed to respond to you and, where a care relationship begins, as required by New York nursing practice recordkeeping standards. You may ask us to delete information related to an inquiry that did not result in care by contacting us using the information below.

- You can ask us to correct or delete inquiry information that did not lead to a care relationship
- You are never required to submit detailed health information through this website, phone is always available as an alternative

Children's privacy
