A medically stable patient or family who needs additional RN care, caregiver preparation, discharge instruction support, or help organizing questions and next steps after discharge, a new diagnosis, or a change in health.
How to refer
Contact HVPN to discuss fit. No PHI is needed for an initial conversation. Provide the patient or authorized family member with HVPN’s contact information; HVPN does not initiate contact. Initial response is typically the same business day.
